When Your Favorite Sleeping Position Suddenly Becomes a Problem

Many side sleepers have used the same sleeping position for years without giving their hips any thought.

Then something changes.

You lie on your usual side and begin feeling an ache over the outside of the hip. At first, it may take several hours to develop. Eventually, the discomfort appears almost as soon as you settle into bed.

So you roll onto the other side.

That works temporarily—until the painful hip begins aching even though it is now on top.

Before long, bedtime becomes a nightly process of arranging pillows, changing positions, and trying to find a place where the hip will leave you alone.

Patients often describe the pain by pointing directly to the bony prominence on the outside of the hip.

That location gives us an important clue because pain on the outside of the hip frequently behaves very differently from pain originating deep within the hip joint.

Lateral Hip Pain: Understanding What Is Actually Hurting

The prominent bone you can feel on the outside of the upper thigh is called the greater trochanter.

Several important soft tissues are located around this region, including the tendons of the gluteal muscles and bursae that help reduce friction between moving tissues.

For many years, pain in this area was commonly labeled:

“Trochanteric bursitis.”

Bursal irritation certainly can occur, but our understanding of lateral hip pain has become broader.

Many patients have involvement of the gluteal tendons, sometimes with associated bursal irritation. Because several structures and mechanical factors may contribute, clinicians often use the term:

Greater trochanteric pain syndrome, or GTPS.

That terminology matters because treatment should not simply be based on the assumption that every painful outer hip is an inflamed bursa that needs to be rested.

Why Side Sleeping Can Be So Uncomfortable

Imagine lying directly on a sore spot for several hours.

That is essentially what happens when sensitive tissues around the greater trochanter are compressed against the mattress.

During the day, the hip experiences constantly changing forces as you:

  • Walk
  • Sit
  • Stand
  • Climb stairs
  • Bend
  • Change positions

During side sleeping, the pressure can remain relatively concentrated.

This helps explain a common pattern:

“I can walk around all day, but I can’t lie on that hip for more than 20 minutes.”

The hip may tolerate movement better than prolonged direct compression.

This is why some patients feel substantially more comfortable when they place a pillow between the knees and thighs.

The pillow is not curing the hip condition.

It is simply changing the position enough to reduce sustained irritation.

That distinction is important: sleep modification can help control symptoms while the underlying problem is being addressed, but it is not necessarily the treatment itself.

Greater Trochanteric Pain Syndrome Is Often a Load Problem

Lateral hip pain frequently develops without one dramatic injury.

A patient may say:

“I didn’t do anything to it. It just started hurting.”

But when we examine the weeks preceding the onset, something may have changed.

Perhaps the patient:

  • Began walking farther
  • Increased treadmill incline
  • Started a new exercise program
  • Returned to running
  • Took a vacation involving extensive walking
  • Began climbing more stairs
  • Increased lower-body workouts
  • Changed work demands

The gluteal tendons help control the pelvis and hip during weight-bearing activity. Like other tendons, they need sufficient capacity to tolerate the loads placed upon them.

When activity increases faster than that capacity, symptoms can develop.

The patient did not necessarily “tear” something.

The relationship between load and capacity may simply have changed.

The Gluteal Tendons Do More Than Move the Leg

Most people think of the gluteal muscles primarily as muscles that move the hip.

They also play an important role in controlling the pelvis while you stand and walk on one leg.

Walking is essentially a repeated series of brief single-leg positions.

Every time you take a step, one leg must support the body while the other moves forward.

The lateral hip muscles help control that process.

This is why a patient with lateral hip pain may notice symptoms during:

  • Walking
  • Stair climbing
  • Standing on one leg
  • Getting out of a car
  • Certain exercises

A complete evaluation therefore looks beyond whether the hip hurts when someone presses on it.

We want to understand how the hip handles load.

The Relationship Between Compression and Capacity

This is one of the most useful concepts for understanding lateral hip pain.

A symptomatic tendon may be irritated by two different challenges:

Compression — such as lying directly on the hip for hours.

Load — such as walking, stairs, running, or repeated single-leg activity.

Some patients are much more sensitive to one than the other.

One person may walk three miles comfortably but cannot sleep on the hip.

Another may sleep reasonably well but develop pain after ten minutes of walking.

A third may struggle with both.

The goal is not simply to find the painful spot.

It is to determine what the tissues currently cannot tolerate and progressively restore that capacity.

Not Every Painful Hip Is Greater Trochanteric Pain Syndrome

Where the patient feels the pain can substantially change the diagnostic picture.

Broadly, hip-region pain may be described in three areas:

Lateral hip pain

Pain over the outside of the hip may bring greater trochanteric pain syndrome and related soft-tissue conditions into consideration.

Groin or anterior hip pain

Pain felt deep in the groin or front of the hip raises greater consideration of the hip joint itself.

Conditions such as osteoarthritis can produce this pattern.

Posterior hip or buttock pain

Pain primarily in the buttock may originate from several sources, including structures around the hip, pelvis, or lumbar spine.

These categories overlap, but they prevent an important mistake:

assuming every pain that a patient calls “hip pain” comes from the same structure.

Hip Arthritis Usually Tells a Different Story

Hip osteoarthritis can certainly interfere with sleep, particularly when symptoms become more advanced.

But the overall presentation often differs from isolated lateral hip pain.

A patient with hip-joint arthritis may describe:

  • Deep groin pain
  • Hip stiffness
  • Difficulty putting on shoes or socks
  • Reduced hip rotation
  • Difficulty getting in or out of a car
  • Pain with prolonged walking
  • Difficulty rising from low seats

Some patients also feel pain toward the outer hip, thigh, or even knee.

That is why location is helpful but not absolute.

The examination tells us whether the hip joint itself appears restricted or provocative.

When Stiffness Becomes an Important Clue

Pain and stiffness are not the same thing.

A patient with lateral hip pain may have relatively good hip motion but experience pain with certain loads or positions.

A patient with significant hip arthritis may say:

“I can’t move that hip the way I used to.”

We may see limitations in:

  • Rotation
  • Flexion
  • Extension

The patient may notice these restrictions during ordinary tasks long before thinking about range of motion.

Putting on a sock becomes harder.

Crossing the legs changes.

Getting out of a low car becomes awkward.

These functional clues help us distinguish a painful soft-tissue problem from a progressively restricted joint.

The Lower Back Can Masquerade as Hip Pain

The lumbar spine is another important part of the differential diagnosis.

Low-back conditions can refer pain into the:

  • Buttock
  • Outer hip
  • Thigh

Nerve-related problems may extend even farther and produce:

  • Burning
  • Tingling
  • Numbness
  • Weakness
  • Symptoms below the knee

This is why treating the exact spot where a patient feels pain is not always sufficient.

If the examination suggests the symptoms are being referred from the lumbar spine, the treatment strategy changes.

Likewise, we should not automatically blame the lower back simply because a patient has both back and hip pain.

The goal is to determine which findings actually reproduce and explain the patient’s symptoms.

The Everyday Activities That Help Clarify the Diagnosis

Instead of asking twenty isolated questions, it can be more useful to understand how the hip behaves during an ordinary day.

A patient might tell us:

“It hurts when I first lie on it, but walking is fine.”

Another says:

“Sleeping is bad, stairs hurt, and after about 20 minutes of walking I start limping.”

Another reports:

“The outside doesn’t really hurt. It’s deep in my groin, and I can barely put my shoe on.”

Those three patients may all say:

“My hip hurts at night.”

But they are describing very different clinical patterns.

We therefore look at several functional domains together:

  • Sleep
  • Walking
  • Stairs
  • Single-leg activities
  • Sitting
  • Dressing
  • Getting in and out of a vehicle
  • Exercise

The pattern across those activities often tells us more than any single provocative movement.

Sleep Position Is Useful Information, Not the Entire Diagnosis

Patients sometimes arrive convinced that their mattress caused the problem.

Others believe they need a softer mattress.

Or a firmer mattress.

Or a new pillow.

A mattress can certainly affect comfort, particularly when direct pressure is provocative.

But if one specific hip has suddenly become intolerant of a sleeping position that was comfortable for years, we should consider the hip itself rather than assuming the bed changed.

Similarly, if the hip hurts with:

  • Walking
  • Stairs
  • Standing on one leg
  • Exercise

in addition to sleeping, changing mattresses is unlikely to address the entire problem.

The sleep position is one part of the story.

Practical Ways to Reduce Nighttime Irritation

While the hip is being evaluated or treated, several simple positioning strategies may improve comfort.

For some patients, that means avoiding prolonged direct pressure on the painful side temporarily.

When sleeping on the opposite side, placing a pillow between the knees and thighs can prevent the top leg from dropping too far across the body.

Back sleeping may also be more comfortable for some people.

The important principle is to use positioning to reduce unnecessary irritation without becoming afraid of the hip.

There is a difference between temporarily modifying a painful position and deciding that the hip is too fragile ever to tolerate pressure again.

The long-term objective is greater capacity, not permanent avoidance.

When Nighttime Hip Pain Deserves Evaluation

Occasional soreness after an unusually active day may settle on its own.

Persistent symptoms deserve more attention when they begin changing normal behavior.

That may include:

  • Repeatedly waking at night
  • Being unable to sleep on either side
  • Developing a limp
  • Losing walking tolerance
  • Increasing difficulty with stairs
  • Progressive hip stiffness
  • Difficulty putting on shoes or socks
  • Increasing weakness

The trajectory matters.

Night pain sometimes worries patients because they have heard that it can indicate serious disease.

The context is important.

Pain that predictably appears when you lie directly on one hip and improves when you change position behaves like a mechanically influenced symptom.

That does not tell us exactly which structure is responsible, but the positional relationship is useful.

We become more cautious when pain is:

  • Severe and unrelenting
  • Progressively worsening
  • Unaffected by position
  • Associated with fever or systemic illness
  • Associated with unexplained weight loss
  • Very different from the patient’s usual musculoskeletal symptoms

Those presentations deserve appropriate medical evaluation.

For the more typical mechanical presentation, our task is different.

We want to determine whether the problem is primarily coming from the lateral hip tissues, the hip joint, the lumbar spine, or another source—and then understand why that region has become less tolerant of the activities and positions the patient needs in everyday life.

The examination begins by determining where the symptoms are actually coming from.

Pain that a patient calls “hip pain” may involve the lateral hip tissues, hip joint, lumbar spine, or another nearby structure. Treating all of those presentations the same way would miss the point of the evaluation.

The history is equally important.

We want to understand whether symptoms developed gradually or after an injury, whether activity recently changed, which sleeping positions provoke the pain, and whether the patient is also losing walking tolerance, strength, or mobility.

The objective is to build one coherent explanation for the patient’s symptoms rather than collecting isolated positive tests.

Distinguishing Lateral Hip Pain From Hip-Joint and Lumbar Problems

One of the most important parts of the evaluation is deciding which clinical pattern fits best.

With greater trochanteric pain syndrome, we may find tenderness over the lateral hip and symptoms associated with compression or loading of the gluteal tendons. Walking, stairs, prolonged standing, or single-leg activities may also reproduce symptoms.

A hip-joint problem may present differently. Reduced rotation, groin pain, stiffness, and difficulty with activities such as putting on shoes or getting into a car can increase suspicion that the joint itself is involved.

A lumbar or neurological source becomes more relevant when the symptoms behave according to spinal position or are accompanied by numbness, tingling, weakness, or pain extending farther down the leg.

Sometimes more than one problem exists.

A patient can have age-related hip arthritis and lateral tendon pain at the same time. Someone can also have lumbar degeneration visible on imaging without the lumbar spine being the primary cause of today’s hip symptoms.

Clinical correlation matters.

Treatment Is About Restoring Capacity, Not Simply Avoiding the Painful Side

For many mechanical lateral hip conditions, the long-term solution is not:

“Never sleep on that side again.”

Temporarily reducing compression may help calm an irritable hip, but the broader objective is to improve the region’s ability to tolerate normal life.

Depending on the diagnosis and examination findings, conservative management may include:

  • Modification of aggravating activities
  • Therapeutic exercise
  • Progressive strengthening
  • Movement or mobility work
  • Chiropractic or manual treatment when appropriate
  • Soft tissue therapy
  • Class IV laser therapy
  • Sleep-position modification
  • Gradual return to walking, exercise, or other activities

The balance matters.

Too much loading can repeatedly aggravate a sensitive hip.

Too little loading for too long can leave the tissues poorly prepared for the activities the patient eventually wants to resume.

Building a Stronger, More Capable Hip

For lateral hip pain involving the gluteal tendons, rehabilitation often becomes a central part of care.

The gluteal muscles need to help control the pelvis during walking and other weight-bearing activities. Rehabilitation may therefore progress beyond simple stretches and eventually challenge the hip in ways that resemble normal function.

Depending on the individual, exercises may address:

  • Hip abductor strength
  • Hip extensor strength
  • Pelvic control
  • Single-leg stability
  • Balance
  • Walking tolerance
  • Stair tolerance

Early exercises may be relatively simple.

As capacity improves, the program can become progressively more demanding.

A patient who simply wants to walk comfortably through the grocery store may require one level of rehabilitation.

A runner, tennis player, or hiker may need considerably more.

The endpoint should reflect the patient’s life.

Why Stretching Is Not Always the Answer

Patients with lateral hip pain are often told:

“Your hip is tight. Stretch it.”

Sometimes mobility work is appropriate.

But aggressive stretching is not automatically helpful for every lateral hip problem.

Certain positions can increase compression around already-sensitive lateral hip tissues. Repeatedly forcing the hip into those positions simply because the area feels “tight” may aggravate some presentations.

This illustrates a broader principle of musculoskeletal care:

A painful area does not automatically need to be stretched.

We first need to determine whether the meaningful problem is:

  • Mobility
  • Strength
  • Load tolerance
  • Compression sensitivity
  • Joint dysfunction
  • Neurological referral
  • Some combination of these

Then treatment can address the actual deficit.

Chiropractic and Manual Care

When clinically relevant restrictions are present, chiropractic or manual treatment may be incorporated as part of a broader conservative plan.

Depending on the examination, treatment may address the:

  • Hip
  • Pelvis
  • Lumbar spine
  • Other mechanically related regions

The purpose is not to “put the hip back in place.”

Nor should every patient with lateral hip pain receive the same adjustment.

For a patient whose primary deficit is reduced strength and tendon capacity, rehabilitation remains essential regardless of how much temporary relief passive treatment provides.

Soft Tissue Therapy and Class IV Laser Therapy

Soft tissue treatment may be used when clinically relevant muscular or soft-tissue findings are present around the hip and surrounding regions.

For selected patients, Class IV laser therapy may also be incorporated as an adjunct to conservative care.

Laser therapy delivers therapeutic light energy to targeted tissues to support normal healing processes.

Neither approach should replace the active component of recovery when strength and load tolerance need to be restored.

A useful way to think about treatment is that passive therapies may sometimes help create a more comfortable environment for recovery, while progressive rehabilitation helps the patient build the capacity needed to stay recovered.

A Smarter Return to Walking and Exercise

Because lateral hip pain frequently relates to load, one of the most important decisions is how quickly activity should increase.

Suppose a patient discovers that the hip becomes painful after approximately 20 minutes of walking.

Repeatedly walking for an hour and producing a significant flare may not be the best way to build capacity.

But stopping all walking for several weeks may not be necessary either.

Instead, the patient may begin with a tolerable amount and progressively increase as the hip responds.

We can track changes in:

  • Distance
  • Duration
  • Post-activity soreness
  • Next-day response
  • Limping
  • Stair tolerance
  • Nighttime symptoms

That provides a much richer picture than asking only whether the hip still hurts.

Sleep Can Become a Functional Outcome

Nighttime symptoms give us another useful measure of recovery.

Imagine a patient initially reports:

  • Pain within 10 minutes of lying on the affected side
  • Three or four awakenings every night
  • Inability to sleep comfortably on either side

Several weeks later, the patient may still occasionally notice the hip during the day but now reports:

  • Sleeping through the night
  • No longer needing multiple pillows
  • Tolerating the affected side for an hour
  • Eventually returning to normal side sleeping

Those changes matter.

A pain scale might improve only a few points while the patient’s actual quality of life improves substantially.

Our Clinical Experience

A particularly common presentation is the patient who initially thinks the problem is entirely their mattress.

They may have already tried:

  • A mattress topper
  • A different pillow
  • Sleeping on the other side
  • Sleeping on their back
  • Putting a pillow between their knees

Some of those changes help.

But then the patient mentions almost casually:

“Actually, it has also started bothering me when I go up stairs.”

Or:

“Now that you mention it, I notice it about halfway through my walk.”

That additional information changes the picture.

The mattress may influence nighttime comfort, but the hip is also showing reduced tolerance during daytime loading.

Conversely, another patient may have excellent strength, normal walking tolerance, and symptoms almost exclusively with direct pressure.

These patients should not automatically receive identical treatment simply because they both say:

“My hip hurts when I sleep on it.”

The pattern matters.

Imaging Should Answer a Clinical Question

Most mechanical lateral hip pain does not automatically require advanced imaging at the first visit.

Imaging becomes more useful when it answers a specific question or when the clinical course does not behave as expected.

X-rays

X-rays may be appropriate when there is concern for:

  • Hip osteoarthritis
  • Fracture
  • Significant bony abnormality
  • Certain structural conditions
  • Persistent symptoms where joint pathology is suspected

MRI or Diagnostic Ultrasound

Advanced imaging may be considered when there is concern for:

  • Significant gluteal tendon injury
  • Other soft-tissue pathology
  • Occult injury
  • Persistent symptoms that remain unexplained
  • Failure to improve appropriately with conservative management

The presence of an abnormality on imaging does not automatically prove that it is the source of pain.

As with many musculoskeletal conditions, imaging findings must be matched to the patient’s history, examination, and function.

When Specialist Evaluation Becomes Appropriate

Some hip presentations require care beyond routine conservative management.

Referral to orthopedics, sports medicine, physical medicine, or another appropriate specialist may be indicated when there is:

  • Significant traumatic injury
  • Suspected fracture
  • Major tendon injury
  • Advanced hip arthritis
  • Progressive weakness
  • Significant loss of function
  • Persistent symptoms despite appropriate conservative care
  • A condition potentially requiring injection, procedural, or surgical management

Progressive lateral hip problems associated with significant tendon dysfunction also deserve appropriate evaluation rather than indefinite symptom management.

Good conservative care includes recognizing when another type of care is needed.

Red Flags: When Hip Pain Needs Prompt Medical Attention

Seek timely medical evaluation when hip pain is associated with findings such as:

  • Inability to bear weight after significant trauma
  • Obvious deformity
  • Severe or rapidly worsening pain
  • A red, hot, markedly swollen joint
  • Fever or systemic illness
  • Unexplained substantial weight loss
  • Progressive neurological weakness
  • Significant unexplained leg swelling
  • Calf swelling or pain accompanied by chest pain or shortness of breath
  • Severe persistent night pain that is not influenced by position or movement

These presentations should not simply be assumed to represent ordinary greater trochanteric pain syndrome.
